Athletic Programs – Apply Now

To apply to any of the SAIIE Athletic Programs you must first begin the academic application on the UW-Platteville website (see above).

To confirm your spot on one of the SAIIE athletic programs you must download and fill out the following forms:

  1. Student Athletic form
  2. Cancellation, refund policy and disciplinary refund policy
  3. Health statement form
  4. Consent Form

All forms related to the SAIIE athletic programs must either be scanned in and emailed to the Planning and Development Director and Coordinator of Sports Programs, Sean Chipres, at seanrc@saiie.com or sent via mail to SAIIE along with the sports fees at the following address:

Interested students should apply as soon as they have determined that the program will meet their personal and academic goals. Qualified candidates are accepted on a first-come, first-served basis. Notification of acceptance is sent approximately 10 days after all application materials have been received.
